New Patients
Complete the appropriate intake form and fax to the office (517-347-8420) to request an appointment. For patients under 18 years, please complete the Child & Adolescent Intake form. For patients ages 18 to 21 years old, please complete the adult intake form. These forms can be found on the "Forms" section. Please fax or mail the completed form to the office. Please note the office must accept the fax, which is typically available from 8:30 am to 4:00 pm daily, but you can call the office to confirm.
There are times when follow up patients will reschedule and there is room for a new patient appointment. If the appointment is less then 48 hours away, the office will call patients on the waitlist starting at the top. However, messages will NOT be left due to the short time until the appointment. Scheduling the first person who answers allows time for pre-appointment paperwork to be completed and for Dr. Kennedy to prepare for the appointment. We understand this can be frustrating for parents who are not able to answer the phone, but please note you will retain your place on the waitlist. If you receive one of these calls, that also means you are toward the top of the waitlist.
